    I haven't really tested this yet but this should work... Using a battle event that is triggered when a certain actor's HP is 0 or below, activate the game over scene. Further more I believe you could use Yanfly Base Troop Events to help... I'm sure there are other ways but that seems like...

    That looks normal... try to change transfer event to Malaise (11, 37) instead of (11, 36)

    When you are transferred back be sure to set it once they walk out not on the door. ...but if you must and really want it done then the door will have to remain open until they move down... or else you end up with a situation like your screenshot. Another thought/idea is to use the "through"...
